To our patients,

We know that while the state, country, and world is staying at home, the dermatologic needs of our patients are still concerning and pressing. We are here and ready to help address your dermatology needs using our new TeleDermatology service; we believe you will find both convenient and easy to use.

To get started, you simply need to be registered in our system. If you are an existing patient to our practice, just call our front office (541-200-2777) and request TeleHealth access. You will then receive an email with instructions on how to download and log-in to our iPhone or Google Play Store App, PocketPatientTM. (If you do not receive an email, make sure to check SPAM or JUNK folders!)

 

Note: If you are new to our practice, we can easily get you established with our clinic to ensure your insurance will cover your TeleDermatology care. We can quickly send you our new patient paperwork via Adobe DocuSign, and provide directions for submitting a government-issued photo ID and insurance card(s). 

Once, you are able to log-in to PocketPatientTM, the attached instruction guide will walk you through how to submit your e-Visit request to one of our Providers. Our Medical Assistants and front office staff will then reach out to you via text on the App or by phone to schedule you with a Provider for a face-to-face video visit.

Our entire staff remains prepared to help with any questions, including how to obtain and use our new TeleDermatology access. We are standing by to assist you in these trying times and hope you, your family, friends and colleagues are well!
